Minors

A parent or legal guardian must sign all new patient paperwork/consents and must attend all appointments with minor children having any treatment other than exam or regular cleaning appointment. This includes minors who can drive themselves. Also please note, dropping minors off or leaving the office while they are under our care is not permitted. If you have any questions or concerns please don’t hesitate to talk to us.

Your scheduled appointment time has been reserved specifically for you. Please arrive on time to all appointments. If you must reschedule, we request a 24 hours notice. This is an important courtesy to other patients and our staff. Failure to comply with recommended treatment schedules may prevent us from achieving your healthcare goals. Missing two or more appointments without notifying us 24 hours in advance may result in a $50.00 fee or dismissal from our practice. Please note, if you dismiss yourself voluntarily at any point, we will no longer render care, effective at dismissal time. If we dismiss a patient from our practice, we always offer emergency care for 30 days following notification so to allow time for the person to find another dental provider.
